Our post acute care rehabilitation programs are designed to transition patients from the hospital back to their regular lives safely, quickly and with confidence. And when it comes to care plans, one size never fits all. We respect every person’s individuality, and tailor care to their needs.
Here in the barn quilting capital of the world, in a small city wrapped by the Wolf River, we treat residents and patients like family and embrace their families as our own. There’s a large recreation area for friends and family, an on-site beauty salon, our private chapel and warm, welcoming smiles everywhere from team members and the volunteers who come to visit.
