Fysio- en Manuele Therapie D.R. Leer bestaat sinds november 2002. Wij verzorgen hoogwaardige fysiotherapeutische zorg waarbij de patiënt centraal staat. Wij behandelen op verwijzing van een arts of via directe toegankelijkheid (=zonder verwijzing). Alleen met uw toestemming rapporteren wij aan uw huisarts of aan de verwijzend arts. Sinds 1 januari 2006 zijn fysio- en manuele therapeuten direct toegankelijk geworden (afgekort DTF= Directe Toegankelijkheid Fysiotherapie). Dit betekent dat U als patiënt geen verwijzing meer nodig hebt van huisarts of specialist voor fysiotherapie of manuele therapie. Bij aanmelding vragen wij uw naam, adres en woonplaats (NAW) gegevens. Neem dus uw identiteitsbewijs, verzekeringsbewijs en eventuele verwijsbrief mee.Wij behandelen bij voorkeur individueel, niet in groepsverband. Bij ons staan de patiënt en zijn/ haar aandoening centraal. Wij bieden naast algemene fysiotherapie ook , fysiotherapie bij de ziekte van Parkinson, orthopedische geneeskunde volgens Cyriax, McKenzie- rugscholing, sportblessurebehandeling en revalidatietherapie aan.
Wij behandelen indien nodig ook aan huis.
Wij zijn BIG- geregistreerd en staan ingeschreven in het Centraal Kwaliteitsregister (CKR) van het KNGF. Wij zijn aangesloten bij KNGF , NVMT en bij ParkinsonNet Roosendaal.
Wij hebben in 2010 en 2011 deelgenomen aan Kwaliefy, het landelijk kwaliteitsonderzoek in de eerstelijns fysiotherapie dat door het KNGF en het UMC St. Radboud Nijmegen is ontwikkeld. Wij zijn hierin door onze patiënten erg goed beoordeeld: de gemiddelde score over alle items (zoals resultaat van de behandeling, accommodatie, bejegening door therapeut) in onze praktijk bedraagt 9,0.
In uw verzekeringspolis staat beschreven welke vergoeding voor fysiotherapie of manuele therapie u hebt verzekerd: deze vergoedingen vindt u in de polisvoorwaarden van de aanvullende verzekering. U kunt hiervoor ook de website van uw zorgverzekeraar raadplegen.
