Providence Hood River Memorial Hospital Anticoagulation Clinic provides pharmacist-led anticoagulation management, primarily for patients taking warfarin (Coumadin) and similar medications. The clinic offers a one-on-one initial consult to review medications, dosing, diet and other factors that affect INR; a point-of-care finger‑prick INR test with immediate results; dose adjustments during the same visit; and shorter follow-up visits. The service is intended to increase time in therapeutic INR range, improve patient safety, and allow patients’ physicians to focus on other health concerns.
