Functional Assessment Rehab started in 1986 as Dell C. Felix, PT & Associates and in 2007 became Functional Assessment Rehab. We have focused on the need to provide a service to the community that focuses on functional abilities and return-to-work options. We strive to look at the individual as a whole and how the body functions together, not just the injured body part. By doing this we have helped many individuals return to their full work duty as well as their recreational activities.
We have 3 physical therapists and no support staff, which means that the patients are receiving the direct care from the physical therapist. The therapists have a great working relationship with each other and are able to help each other to be creative with exercises to maximize the patients needs to get back to their prior level of functioning.
We offer accept BlueCross BlueShield, Humana, all Workers Comp, and Cash pay options.
