The Silver Chain Group are known for delivering palliative care, community health and aged care, aged care facilities services in Perth and across Australia. They also have a range of services such as specialist nursing, palliative care, home care and support services, home hospital and allied health services, such as physiotherapy, podiatry and speech pathology to compliment their aged care services.
Our goal is to help people of all ages to receive the palliative and aged care services they need so they can continue to live independently at home. We also offer aged care facilities for those who require a bit more attention. We currently deliver our services in Western Australia, South Australia, Queensland, New South Wales and Victoria.We also have a range of equipment including personal alarms and other technology-based support to assist clients of all ages.
